Vieux-Ruffec liegt etwa 47 Kilometer nordnordöstlich von Angoulême. Vieux-Ruffec wird umgeben von den Nachbargemeinden Le Bouchage im Norden, Benest im Nordosten, Champagne-Mouton im Osten und Südosten sowie Nanteuil-en-Vallée im Süden und Westen.
