Die 16. Mille Miglia, auch Mille Miglia, Brescia-Roma-Brescia, fand am 23. und 24. April 1949 statt und führte über 1.602 km von Brescia nach Rom und wieder zurück nach Brescia.

Der Rennverlauf
Die Öffnung der vor dem Zweiten Weltkrieg in erster Linie Profirennfahrern vorbehaltenen Rennveranstaltung für Amateure setzte sich 1949 fort; mit letztlich negativen Folgen für das Rennen selbst. 1949 nahmen 301 Fahrzeuge das Rennen auf und durch die obligaten Zweierteams 602 Rennfahrer. Diese Zahl entsprach beinahe der Anzahl der Italiener, die 1949 eine Rennlizenz besaßen. Damit nahm aber die Gruppe an Piloten überhand, die so gut wie keine Erfahrung bei langen Straßenrennen hatten. Nur mehr eine Handvoll Spitzenpiloten ging 1949 an den Start.
Der italienische Motorsportverband und die Organisatoren der Mille Miglia wollten mit dieser Strategie vielen Fahrern den Einstieg in den Motorsport ermöglichen. Viele Fahrer waren aber überfordert, ihre Fahrzeuge reine Straßenwagen und schlecht vorbereitet. Die technische Abnahme blieb bei der Menge an Teilnehmern lückenhaft, sodass Wagen an den Start gingen, die schon im normalen Straßenverkehr eine Gefahr darstellten. Die Folge waren, trotz entschärfter Route, eine Fülle an Unfällen, von denen einer tödlich endete. Die beiden Briten James Cohen und Reg Hignett verunglückten mit ihrem Healey 2400 ungefähr 35 Kilometer nach dem Start in Brescia auf einer Brücke. Während Hignett sofort tot war, kam Cohen mit schweren Verletzungen in ein Krankenhaus. Er starb nach dem Rücktransport nach Großbritannien wenige Wochen später an den Folgen seiner schweren Verletzungen[1].
Positiv aufgenommen wurde die Änderung der Startprozedur. Ab jetzt konnte man die Startzeit als Startnummer auf den jeweiligen Fahrzeugen ablesen. Den Zuschauern war es dadurch möglich, die bisherige Fahrzeit der Teilnehmer einfach zu berechnen und so immer über den Rennverlauf Bescheid zu wissen. Die Startzeit des späteren Siegers Clemente Biondetti war um 06:24 Uhr; daher seine Startnummer 624. Der Elftplatzierte Luigi Fagioli hatte die Nummer 424 und war mithin um 04:24 Uhr ins Rennen gegangen.
Das Rennen war von Anfang an eine klare Angelegenheit der Werks-Ferrari. Einzig Franco Rol konnte im privaten Alfa Romeo 6C 2500 Competizione Berlinetta den Ferrari-Piloten Paroli bieten und wurde am Ende Dritter in der Gesamtwertung. Clemente Biondetti gewann nach 1938, 1947 und 1948 seine vierte Mille Miglia, die dritte in Folge, und wurde zum erfolgreichsten Piloten des 1000-Meilen-Rennens. Zweiter wurde sein Teamkollege Felice Bonetto, der in Rom noch geführt hatte.
