Saint-Vivien-de-Blaye liegt etwa 29 Kilometer nördlich von Bordeaux. Umgeben wird Saint-Vivien-de-Blaye von den Nachbargemeinden Saint-Christoly-de-Blaye im Norden, Civrac-de-Blaye im Osten, Pugnac im Süden sowie Teuillac im Westen und Südwesten.
