Sainte-Gemme liegt etwa 16 Kilometer ostnordöstlich von Bressuire. Umgeben wird Sainte-Gemme von den Nachbargemeinden Luché-Thouarsais im Norden und Westen, Saint-Varent im Osten, Pierrefitte im Süden sowie Geay im Südwesten.
