Die Football League Fourth Division 1990/91 war die 33. Spielzeit der vierthöchsten englischen Fußballliga. Die vier bestplatzierten Vereine FC Darlington, Stockport County, Hartlepool United und Peterborough United stiegen direkt in die Football League Third Division auf. Die Vereine auf den Plätzen fünf bis acht spielten im Play-off-Modus den fünften Aufsteiger aus. Hierbei setzte sich Torquay United durch.

Absteiger gab es diese Saison keinen. AFC Wrexham als Tabellenletzter vermied den Abstieg, da die Teilnehmerzahl für die folgende Spielzeit, in allen vier Ligen zusammen, von 92 auf 93 erhöht wurde.
