Er entsteht als Ausfluss aus dem See Mierasjärvi und mündet nach ca. 50 km beim Ort Utsjoki in den Tanaelva (Tenojoki). Der Fluss verläuft in Süd-Nord-Richtung. Dabei durchfließt der Utsjoki mehrere langgestreckte Seen, die Landschaft hat einen Mittelgebirgscharakter.
